Tamaqua Area Middle School
Dear Parent/Guardian:
As Certified School Nurses and RN's your child's health is very important to us.
Physical examinations are required in Grades K, 6, and 11. The physical exams are designed to examine your children and make recommendations to you for treatment of any physical fault found during the physical examination. Dental exams are required in Grades K, 1, 3 and 7. Pennsylvania school health law requires that all students have height and weight measurements, BMI calculation sent home and vision screening done annually. Hearing screening is done in Grades K, 1, 2, 3, 7, and 11 and as needed if a problem arises. Scoliosis screenings are done on students in Grades 6 and 7.
In addition, we monitor immunization records every 60 days and request that you provide us with dates of any boosters received after you initially submitted information at the time of school registration.
We do not treat illness or injuries, as that should be done by your family physician. However we do assess your child and provide first aid and in an emergency we will be guided by the "Emergency Information" form completed by you. Please notify us immediately of any change of information on the emergency form. (Example: change of address, change of telephone number, or change of medical information).
We realize that at times it may be necessary for your child to receive medication during school hours. However, school nurses may not administer medications including over the counter medicines to students without a specific written order from your health care provider. If medication is given at school please be sure that the medication is in a pharmacy labeled container and bring the medicine to the nurse.
If a contagious disease is present, we will be guided by the rulings of the local Board of Health concerning exclusions and return to school.
We have noticed that if a child has academic or other difficulty in school, he/she sometimes experiences physical complaints and visits the Health Room frequently. We will notify you if your child misses valuable class time and makes frequent visits to the Health Room. Please stress the importance of classroom attendance!
Thank you for your continued cooperation in helping us to make our school a better place in which to live and grow.
Sincerely,
Christine Kostecky, RN
