Good morning,

Below you will find information regarding another offering for free sports physicals.  Any student that has NOT received a sports physical this school year and plans on participating in a spring sport - must have a physical before playing.

Here is the link to the physical form - which must be taken to the physical.

Any student who has already had a physical - must complete section 8 - parent re-certification form in order to participate.

Time - 5pm-7pm

Location - Panther Valley SD

Slots avail - 80 (10 athletes every 15 min)  Able to adjust if needed
