    Walk-ins will not be accepted.

    Mandated Dental Exams

    By state mandate, ALL students must show evidence of receiving a dental exam during Kindergarten or 1st grade and in 3rd grade. You can have your family dentist complete the required form and return it to the school nurse OR you can have your child evaluated by the school dentist at no cost to you. The dental exam completed by the school dentist is just a screening where the dentist will look at your child's teeth and gums.

    If you would like the school dentist to complete the exam, please return this dental form to the school nurse.

    Who is Eligible? EVERYONE!

    We believe all children are deserving of accessible dental care. We have a dedicated team to help.

    -CHILDREN CURRENTLY UNDER THE CARE OF A DENTIST ARE NOT ELIGIBLE TO SIGN UP UNLESS THEY WILL BE DISCONTINUING CARE WITH THEIR CURRENT DENTIST.

    -Children who have Medical Assistance, AmeriHealth Caritas, Avesis, Geisinger, United HealthCare, Aetna Better Health, Gateway, Capital Blue Cross, Cigna, Delta Dental, Guardian, MetLife and United Concordia.

    -No Insurance? Or have an insurance we do not pariticipate with? We can help! We believe all children are deserving of accessible dental care. We have a dedicated team to help.     TO HAVE YOUR CHILD SEEN FOR SERVICES IN THE DENTAL VAN: PLEASE COMPLETE THE FOLLOWING FORM AND RETURN TO THE SCHOOL IMMEDIATELY, OR CONTACT YOUR CHILD'S SCHOOL NURSE.
